Job summary

Comprehensive Rehab Consultants is seeking a PM&R Clinician (NP/PA) to deliver specialized PM&R services at designated SNFs or post-acute centers in Middleborough, MA. You will work with the post-acute therapy team and interdisciplinary care partners to improve patient independence.

In this full-time W-2 role, you will collaborate with PCPs, therapists, and facilities, manage conditions common in SNFs, and participate in ongoing education and culture-building activities.

Qualifications

• Nurse Practitioner/APRN/Physician Assistant license required.
• Massachusetts license required.
• Ability to work in the U.S.
• Post-acute/SNF experience preferred.
• Strong patient care mindset with empathy.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and collaboration.
• Collaborate with PCP teams for SNF/post-acute conditions management.
• Work with PT/OT/SLP teams to improve function and quality of life.
• Follow patients to track progress in therapy.
• Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
• Be part of SNF team, attend meetings, bridge therapy and internal medicine.
• Engage in culture building, monthly lectures, regional meetings, and dinners.
