Comprehensive Rehab Consultant seeks a clinician to provide PM&R/physiatry services in post-acute settings. You will lead clinical care within SNFs, collaborate with PCPs and therapy teams, and help manage conditions common in post-acute populations.

Ideal candidates have SNF/post-acute experience, strong autonomy, and a compassionate approach. The role offers a 4-day work week, 8:00–17:00 schedule, and a competitive salary with bonus options within Edmond, OK.

Qualifikationen

• Post-acute/SNF experience preferred.
• Ability to work autonomously and deliver high-quality patient care.
• Strong interpersonal skills and empathy with patients and facility partners.

Aufgaben

• Provide PM&R/physiatry services as a rehab consultant.
• Represent CRC with clinical leadership in SNF facilities.
• Collaborate with PCPs and therapy teams to manage common post-acute conditions.
• Follow patients through therapy to track progress and outcomes.
• Bridge gaps between therapy and internal medicine through multidisciplinary participation.
• Participate in ongoing education and regional team activities.
