Comprehensive Rehab Consultants (CRC) seeks a NP/PA PM&R Clinician to deliver specialized Physiatry services in a post-acute SNF setting. Location spans Westerly, RI and New London, CT, with a 32-hour W-2 role.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• Connecticut and/or Rhode Island license.
• Ability to work in the U.S.
• Experience in post-acute space preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with interdisciplinary care team and PCPs.
• Support patient function improvement in SNF/post-acute settings.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Follow patients to track progress and address barriers to therapy.
