Job summary

Ascension Medical Group Old Town Schaumburg is seeking a Board Certified Nurse Practitioner for a full-time outpatient Family Medicine position in Schaumburg, IL. The role includes direct patient care, support from a large physician-APP team, and a strong focus on preventive care and family medicine across all ages.

Base salary ranges from $112,000 to $135,300 annually, with RVU/quality incentives, CME allowance, and comprehensive benefits.

Qualifications

• BLS Provider within 30 days of hire date (AHA or Red Cross accepted).
• Illinois Nurse Practitioner license/credentialing obtained prior to hire date.

How you’ll make an impact in this role

• Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care.
• Provides direct treatment and management of health conditions via referral to other healthcare providers or community resources.
• Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources.
• Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
• Preferred Credential(s):
• Registered Nurse.

Education

• Master's degree of Nursing required.
