Job summary

AIDS Healthcare Foundation is seeking a Non-Medical Care Manager to coordinate and support access to HIV care and related services. You will complete initial assessments, develop individualized care plans for psychosocial needs, and connect clients with housing, food, and clothing resources while working closely with providers and the care team.

You will conduct ongoing retention calls to prevent gaps in care, advocate for clients, and help enroll eligible clients in accessible programs,

Qualifications

• High School Diploma is required.
• Bachelor degree in health or human services-related field preferred.
• Minimum of 1 year Case Management experience preferred.
• Knowledge of Ryan White Services preferable.

Responsibilities

• Complete Initial Assessments and develop Individualized Care Plans for psychosocial needs and goals.
• Coordinate housing, food, and clothing referrals and facilitate referrals to support services.
• Conduct ongoing retention calls to minimize gaps in retention efforts and promote coordination across divisions.
