Job summary

AID Atlanta is seeking a Medical Case Manager to support people living with HIV/AIDS by coordinating services, assessing needs, and creating individualized care plans. You will work directly with members to remove barriers to care and ensure adherence.

The role involves case management duties, home or hospital visits as needed, and regular communication with the team to optimize service delivery. Prior HIV experience is preferred but not required.

Qualifications

• Bachelor's degree required in social work, psychology, sociology, human services or related field.
• 1-2 years of experience in social work/case management or similar field.
• Experience in HIV field preferred but not required.
• Must pass criminal background check.

Responsibilities

• Provide direct service coordination and support to people living with HIV/AIDS.
• Manage a caseload of up to 50 members.
• Document all encounters within 72 hours in the EMR system.
• Develop and update Individualized Service Plans (ISP) for each member.
• Educate members on case management policies, rights and responsibilities.
• Coordinate with physicians, nurses and other staff to execute care plans.
• Attend monthly interdisciplinary meetings with medical providers.
• Maintain up-to-date community resources and participate in agency activities.
