Job summary

AHF is seeking a Medical Care Manager to coordinate care within the Ryan White program, conducting Needs Assessments, developing Action/Care Plans, and educating patients on diagnoses and medications. You will facilitate referrals and coordinate with providers and community resources.

You will work at the Healthcare Center, advocating for clients and supporting connections to housing, food, and services to improve health outcomes and viral suppression.

Qualifications

• 1 year of Case Management experience preferred.
• Knowledge of Ryan White Services preferable.
• Ability to educate clients about diagnoses and medications.

Responsibilities

• Complete Needs Assessments and develop Action/Care Plans.
• Provide ongoing education to patients about diagnoses and medications.
• Develop referrals, support, and assistance for related needs.
• Collaborate with providers and community programs to access housing, food, and services.
• Contribute to program deliverables under the Ryan White contract.
