Job summary

AIDS Healthcare Foundation seeks a Medical Care Manager to help Ryan White clients access services, develop individualized care plans, and coordinate care across providers. You will assess needs, arrange referrals, and support adherence to complex HIV treatments while addressing social determinants of health.

Qualifications

• Master’s degree in Social Science, Social Work, or Human Services.
• A minimum of 3 years of Case Management experience is preferred.
• Knowledge of Ryan White Services is preferable.
• Knowledge of community resources.
• Competence in Cultural and linguistic Diversity.

Responsibilities

• Provide Needs Assessments and develop Action/Care Plans.
• Coordinate care for PLWH and ensure access to services.
• Conduct referrals, support, and retention efforts across divisions.
• Verify ACA eligibility and enroll clients as appropriate.
• Engage clients to promote viral suppression and adherence to treatments.
