Job summary

Sweetwater Care is seeking an LVN with nursing management experience to oversee care delivery in a long‑term care setting. You will ensure medication administration and treatments comply with policy, coordinate rounds with physicians, and support a robust infection control program.

The role requires California LVN licensure (compact license a plus), long‑term care experience, and proficiency with PointClickCare charting.

Qualifications

• Must be licensed as LVN in California; compact license preferred.
• One year of nursing management experience preferred.
• Experience in long-term care and PCC charting is required.

Responsibilities

• Demonstrates knowledge of Infection Control Manual and applies it.
• Supports Quality Improvement Program implementation.
• Uses Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (admissions, discharges, LOA).
• Educates patients and families.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
