Job summary

Sweetwater Care is seeking an experienced LVN nurse manager to oversee clinical operations and patient care in a long-term care setting. The role emphasizes infection control, quality improvement, and coordination with physicians.

The ideal candidate has a California LVN license, a compact license for needed states, and at least one year of management experience in long-term care, with proficiency in PointClickCare.

Qualifications

• Must hold California LVN license; compact license valid for compact states.
• Minimum 1 year nursing management experience; long-term care experience preferred.
• Experience with PointClickCare charting.

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in Quality Improvement Program implementation.
• Uses Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admissions, discharge).
• Conducts family and patient education.
• Administers medications and treatments timely per facility policy.
• Receives, transcribes, and carries out physician orders per Nurse Practice Act.
• Makes rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
