Job summary

Jackson Creek Post Acute is seeking a Certified Nursing Assistant for full-time night shifts to provide routine daily nursing care per the resident care plan. You will identify residents, document treatments, and report any changes to the Nurse Supervisor while maintaining resident safety, privacy, and comfort.

The role requires Missouri CNA licensure, ability to follow care plans, and teamwork with nursing staff.

Qualifications

• Must be a licensed Certified Nursing Assistant in Missouri.
• Ability to read and follow care plans.
• Good communication and teamwork required.
• CPR preferred.

Responsibilities

• Provide routine daily nursing care per care plan.
• Identify residents before procedures using wristband/photo card.
• Document entries accurately in flow sheets and records.
• Report changes in residents' condition to the Nurse Supervisor/Charge Nurse.
• Maintain resident privacy and confidentiality in all communications.
• Assist residents with transfers, mobility, and activities of daily living.
• Respond to resident calls promptly and check on needs regularly.
• Follow established policies and procedures and cooperate with other staff.
