Job summary

Jackson Creek Memory Care is seeking a CNA - PRN to provide routine daily nursing care to residents under the supervision of licensed staff. The role requires following care plans, reporting changes promptly, and supporting a warm, respectful environment for residents and families in a memory care setting.

Qualifications

• Must be able to read, interpret and follow care plans and clinical instructions.
• Excellent communication with residents, families, and staff.
• Ability to document accurately and timely in charts and records.

Responsibilities

• Provide routine daily nursing care under the care plan.
• Identify residents before administering treatments or meals.
• Record entries clearly on flow sheets and charts.
• Report changes in residents’ condition to Nurse Supervisor promptly.
• Maintain confidentiality of resident information and safety practices.
