Job summary

Jackson Creek Memory Care is seeking a Licensed Certified Nursing Assistant to provide routine daily nursing care per residents' assessment and care plans. You will greet residents, escort them to rooms, assist with daily living activities, and ensure a warm, calm unit environment.

Responsibilities include accurately recording observations, reporting changes to the Nurse Supervisor, and following facility policies on safety, privacy, and patient care during all shifts.

Qualifications

• Must possess at least 10th grade education.
• Must be a licensed Certified Nursing Assistant per state laws.
• CPR certification preferred.

Responsibilities

• Provide routine daily nursing care and services per resident plans.
• Identify residents by wristband/photo before treatments or meals.
• Report changes in resident condition to Nurse Supervisor promptly.
• Record entries accurately on flow sheets, notes, and charts.
• Maintain confidentiality of resident information and report disclosures.
• Assist residents with daily living activities (dental care, bathing, dressing).
• Assist with moving, positioning, and transporting residents as needed.
• Help prepare residents for medical tests and examinations.
• Observe for skin breakdown and notify of any injuries or concerns.
• Participate in nursing shift reports and follow policies.
