Job summary

Jackson Creek Memory Care is seeking a weekend CNA to provide routine nursing care and daily living assistance to residents according to their care plans. You will identify residents before treatments, record observations, and support staff during shift handoffs in a memory-care environment.

You will assist with daily activities, monitor residents for changes, and maintain a safe, warm, and comfortable unit. CPR is preferred and registration as a CNA is required.

Qualifications

• Must possess, as a minimum, 10th grade education.
• Must be a licensed Certified Nursing Assistant (CNA).
• CPR certification preferred.

Responsibilities

• Provide routine daily nursing care per resident assessment and care plan.
• Identify residents before procedures and record information accurately.
• Assist with daily activities (dressing, bathing, feeding) and hydration.
• Monitor residents and report changes to Nurse Supervisor/Charge Nurse.
• Maintain privacy and protect resident information per policies; report breaches.
• Assist with transporting residents to appointments and activities.
• Keep rooms clean, equipment ready, and beds made as required.
• Observe for skin breakdown and report any injuries or concerns.
