Chapters Health System in Florida is seeking an RN Nights and Weekends to provide after-hours field visits to patients, families, and community partners. You will establish and maintain positive relationships and coordinate care across settings.

This role requires an active RN license, 1 year nursing experience (hospice/hospital preferred), EMR/EHR proficiency, a valid driver's license, and strong communication and clinical skills for fast-paced environments.

Qualifications

• Current RN license in the state where employed.
• Minimum of one year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Ability to communicate in English; Spanish preferred.
• Valid driver’s license and automobile insurance; reliable transportation.
• Excellent decision making and problem-solving skills.
• Strong time management and prioritization abilities.
• Ability to work in a fast-paced, cooperative team environment.
• Ability to perform essential nursing functions including lifting.
• Active BLS for healthcare professionals.

Responsabilités

• Makes after-hours visits to patients in home, assisted living, nursing facility, or hospital; provides compassionate service.
• Assesses patient/family needs and identifies appropriate nursing interventions.
• Provides direct care as part of IDG, addressing psychosocial, spiritual, cultural, physical, and biological needs.
• Demonstrates clinical skills in case management, IV therapies, symptom management, and patient/family relationships.
• Prioritizes, consults, and collaborates with others to ensure quality and continuity of care.
• Educates patient, family, caregivers about disease, care, safety, and interventions.
• Reports changes in condition to IDG or health professionals.
• Attends patient deaths; prepares body for transport and manages medication disposal.
• Completes documentation per policy and procedure.
• During emergencies, may report to work at designated locations to ensure continuity of services.
• Performs other duties as assigned.
