• Assess learning needs and barriers, tailor patient education to age, culture, and beliefs.
• Document admission assessments, reassessment, plans, interventions, and medication use.
• Utilize the information system to communicate patient status and document changes.
• Coordinate with the care team during rounds and transfers.
