Job summary

SnapCare seeks a Registered Nurse for the night shift at our Oregon, OH long-term acute care and rehabilitation hospital. You will perform nursing assessments within 8 hours of admission, develop individualized care plans, and provide updates during team conferences.

The role includes supervising LPNs and CNAs, accurate documentation, participation in rehab programs, and initiating discharge planning from day one.

Qualifications

• Minimum two (2) years of bedside clinical experience required.
• Critical care experience required.
• BLS certification required prior to first shift.
• ACLS certification required prior to first shift (CIRH only).
• LTAC experience highly desired.

Responsibilities

• Perform nursing assessments within 8 hours of admission.
• Develop and maintain individualized care plans based on assessment data.
• Provide patient status updates for Initial Team Conferences and weekly Re-evaluation Team Conferences.
• Maintain accurate and timely documentation of assessments, interventions, and patient responses.
• Participate in patient rehabilitation programs on the assigned unit.
• Initiate discharge planning from day of admission and provide patient teaching.
• Communicate pertinent information regarding changes in patient status to attending physicians and Charge Nurse.
• Provide direction, training, and supervision of LPNs, CNAs, and orientees.
• Assume charge responsibility following orientation period as requested.
