Job summary

Reserve Care (formerly Hospice of the Western Reserve) seeks an experienced RN for the 3rd shift at the Lakeside Facility in Cleveland, OH. The role involves providing prescribed medical treatment and personal care in collaboration with the transdisciplinary team, using the Nursing Process to implement the Plan of Care.

You will participate in care conferences, on-call rotation, discharge planning, and patient-family education, while upholding our standards of compassion and quality end-of-life

Qualifications

• Licensed RN in State of Ohio.
• Experience in hospice, home health, oncology, or pain and symptom management preferred.
• Current CPR certification.
• Must have and maintain a valid driver’s license with acceptable driving record.
• Hospice and Palliative Care Certification desired; required within first 2 years.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients with the transdisciplinary team.
• Coordinate Plan of Care implementation using the Nursing Process.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Monitor and manage pain and symptoms and communicate with physicians.
• Participate in care conferences, discharge planning, and on-call rotation.
• Educate patients and families; maintain plans and reports for hospice care.
