Job summary

Reserve Care (formerly Hospice of the Western Reserve) seeks an RN for the 3rd shift at the New Lakeside Facility in Cleveland, OH. This regular part-time role delivers prescribed medical treatment and personal care in collaboration with the transdisciplinary team, following the Plan of Care.

You will assess needs, provide intermittent skilled nursing, manage pain and symptoms, and participate in care conferences, discharge planning, on-call rotation, and coordination with volunteers.

Qualifications

• Licensed RN in Ohio
• Current CPR certification
• Must have and maintain a valid driver’s license

Responsibilities

• Provide intermittent skilled nursing services per physician direction and Plan of Care
• Assess physical, psychosocial, and spiritual needs of patient and family
• Coordinate Plan of Care with transdisciplinary team and arrange follow-ups
• Participate in care conferences, discharge planning and on-call rotation
• Maintain patient records and reports related to hospice care
