Qualifications

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge needs.
• Formulate and adjust care plans with input from patient, family and health team.
• Deliver direct patient care and coordinate activities of the care team.
• Evaluate patient progress and adapt plans as needed.
• Document nursing care accurately in the patient record.
• Provide care based on best available evidence and participate in unit/department leadership.
