Job summary

• Assess, plan, implement and document nursing care according to policy.
• Deliver care with professional communication and patient-centered focus.
• Collaborate with multidisciplinary team to meet patient needs.

Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulates a plan of care with discharge planning and collaboration with patient/family and health team.
• Implements the plan of care through direct patient care, coordination and supervision of staff.
