Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge needs.
• Plans and initiates a plan of care in collaboration with patients and families.
• Implements and coordinates the plan of care, including delegation and supervision of staff.
• Evaluates patient outcomes and updates care plans as needed and communicates effectively with the care team.
