Qualifications

Responsibilities

• Assess patients across physical, psychological, developmental, sociocultural, spiritual and lifestyle factors.
• Develop and implement individualized care plans and revise as needed.
• Administer medications and treatments safely per physician orders and protocols.
• Educate patients and families about conditions, treatments and self-care.
• Coordinate with medical staff to optimize patient care and teamwork.
• Maintain accurate documentation of care and outcomes in the medical record.
• Ensure infection control and safety standards are followed.
