Qualifications

• High school diploma or equivalent required.
• Graduate of an approved Paramedic course with current certification or RN exemption.
• Minimum 2-3 years of clinical experience.
• Demonstrated ability to work in a collaborative leadership environment.
• Proficiency with EHRs and transfer management systems.

Responsibilities

• Coordinate patient transfers between facilities and units.
• Assess transfer requests and determine appropriate level of care.
• Communicate with physicians, nurses, and staff at both sites.
• Document transfer information accurately in EHRs.
• Coordinate transportation arrangements including ground/air ambulances.
• Monitor bed availability and support bed management.
• Act as a clinical resource for transfer questions.
