• Call Center – Transfer Center – Bed Management experience helpful.

Responsibilities

• Responds to phone calls with urgency and strong customer service.
• Gathers initial information to open new case and prepares clinical information for clinicians.
• Locates resources and coordinates information for patient disposition or admissions.
• Maintains bed availability inventory within the community for non-HCA bed placements.
• Facilitates hospital transfers and transports telephonically and via computer applications.
• Provides updates on status of patient cases and bed placement requests.
• Schedules and prioritizes discharge transports to support patient flow.
• Uses clinical judgment to assess situations and facilitate continuum of care.
