Wellpath / Correct Care Solutions is seeking an LPN for night shift to provide patient care under RN supervision. The role involves medication administration, assessment, and collaborative care in a secure, mission-driven environment.

You will contribute to treatment planning, participate in trainings, and support underserved populations while maintaining quality standards and compliance.

Qualifications

• Graduate from an accredited school of nursing.
• Minimum of one year clinic experience.
• Current LPN/LVN licensure in the state of employment.
• CPR certification.

Responsibilities

• Under RN supervision, assess patients' physical, psychological, and social dimensions in Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed.
• Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols.
• Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques and standard precautions while responding to codes or health emergencies.
• Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting.
• Attends mandatory staff meetings and training sessions, demonstrating commitment to continuous professional development and adherence to policies.
