Wellpath / Correct Care Solutions is seeking a Licensed Practical Nurse (LPN) for night shifts on a PRN basis. You will contribute to patient care under RN supervision, administering medications and supporting treatment plans in a nursing department dedicated to underserved populations.

The role emphasizes collaboration with a multidisciplinary team, ongoing training, and adherence to clinical standards to ensure safety and quality in patient care.

Qualifications

• Graduate from an accredited school of nursing.
• One year clinic experience preferred.
• Current licensure as Licensed Practical/Licensed Vocational Nurse in the state of employment.
• CPR certification required.

Responsibilities

• Under RN supervision, assesses patients’ physical, psychological, and social dimensions and contributes to individualized treatment planning.
• Administers medications, including IV therapy if certified, and performs diagnostic tests per protocols.
• Collaborates in controlled substance counts at shift start and responds to codes as needed.
• Assists in procedures, implements nutrition and therapeutic diets, and documents nursing encounters.
• Attends mandatory staff meetings and training to ensure high-quality patient care.
