Job summary

Signature HealthCARE of Chapel Hill is seeking an LPN/LVN to provide direct nursing care on the FT night shift (7p–7a) in a 108-bed facility. You will administer medications, assist with ADLs, supervise CNAs, and coordinate resident care within the interdisciplinary team.

Requirements include an active LPN/LVN license, CPR/BLS preferred, and 1 year of clinical nursing experience (long-term care preferred). Proficiency with EHR and MS Office is expected. Benefits are highlighted in the posting.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) with dignity and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents' status and notify physicians as needed.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
