Job summary

Signature HealthCARE, LLC is seeking a dedicated LPN/LVN to provide direct nursing care in our Chapel Hill facility. You will supervise aides, manage medications, and uphold patient safety and comfort in a 108-bed environment.

The role emphasizes accurate documentation, collaboration with an interdisciplinary team, and participation in ongoing quality improvement. A Current NC license and at least one year of clinical nursing experience are required, with supervisory experience preferred.

Qualifications

• Active LPN/LVN license in NC.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory/charge nurse role preferred.
• Familiarity with EHR systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor and observe residents, notify physician of changes.
• Develop and review individualized care plans with the team.
• Complete clinical documentation per policy and procedure.
• Coordinate admissions, transfers and discharges per procedures.
• Participate in quality assurance and staff education activities.
• Perform other duties as assigned by the Director of Nursing.
