Job summary

Signature HealthCARE of Chapel Hill is seeking a caring LPN/LVN to provide direct nursing care to residents in a 108-bed facility. You will administer medications, assist with ADLs, supervise junior staff, monitor patient conditions, and contribute to individualized care plans.

The role requires an active NC license and CPR/BLS certification is preferred. Prior supervisory experience and familiarity with EHR systems and state regulations are advantageous.

Qualifications

• Active LPN/LVN license in NC.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and maintain comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shift.
• Monitor residents' condition and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
