Signature HealthCARE, LLC in Chapel Hill, NC, is seeking a full-time LPN for day shifts (7am–7pm) to provide direct nursing care, support residents with daily activities, and supervise nursing staff. The role requires an active LPN/LVN license, CPR/BLS preferred, and at least 1 year of nursing experience, with LTC and supervisory background a plus.

Compensation includes a $5k sign-on, comprehensive benefits, and opportunities for professional growth.

Qualifications

• Active LPN/LVN license in the state of practice with CPR/BLS preferred.
• Minimum one year of clinical nursing experience; LTC experience favored.
• Experience in a supervisory or charge nurse role is a plus.
• Strong observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Excellent verbal and written communication; ability to work independently.

Responsibilities

• Provide direct nursing care, including medication administration and wound care.
• Support residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and aides per shift policies.
• Monitor residents' status and notify physicians of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.
