Job summary

Signature HealthCARE of Chapel Hill seeks a dedicated LPN to join our 108-bed facility on the night shift (7p-7a). You will provide direct nursing care, administer medications, assist with ADLs, and maintain accurate documentation for resident safety and comfort.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the assigned shift.
• Monitor residents’ status and notify physician of changes in condition.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
