Job summary

Sentara Health’s hospice services in South Boston, VA, seeks an LPN for third shift (nights) on a part-time basis (24 hours/week). You will provide direct one-on-one home health care, monitor patient health, administer treatments and collaborate with a team of nurses and physicians.

The role requires a VA LPN license, BLS within 90 days, and a minimum of 2 years of experience, with hospice background preferred. Travel up to 50 miles may be required.

Qualifications

• Trade school nursing program graduate is required.
• LPN license in VA is required.
• BLS certification within 90 days of hire.
• Minimum 2 years of experience; hospice experience preferred.

Responsibilities

• Provide direct one-on-one home health care to patients.
• Monitor patient health, administer treatments and manage medications.
• Collaborate with RNs and physicians to develop care plans.
