Job summary

Sentara Hospice is seeking a Licensed Practical Nurse (LPN) to provide direct, one-on-one home health care in Chesapeake, VA. You will monitor patient health, administer treatments, manage medications, and collaborate with RNs and physicians to deliver compassionate care.

Weekend Nights, 30 hours per week (Sat & Sun, 5PM–8AM), part-time role. Requires LPN license in VA and BLS; hospice experience preferred.

Qualifications

• Requires a Trade School Diploma in Nursing or equivalent.
• LPN license in VA is required; BLS certification within 90 days.

Responsibilities

• Provide direct, one-on-one home health care to patients.
