Job summary

Halifax Health Hospice is seeking a Night/LPN field-based clinical professional to provide direct nursing care to hospice patients. Duties include assessments, documentation, and active participation in the interdisciplinary plan of care under RN supervision.

24/7 coverage across nights, weekends, and holidays with on-call rotations. The ideal candidate will have an LPN license in Florida, BLS and IV certification, and at least 1 year of Medical-Surgical or hospice experience.

Qualifications

• Valid Driver's license with a good driving record; Personal car in good working order
• BLS and IV certification per department requirement
• Minimum 1 year experience in Medical-Surgical Nursing or Hospice experience preferred
• Graduated from an accredited nursing program, must meet criteria for employment by Halifax Health

Responsibilities

• Provide direct patient care to hospice patients under RN supervision
• Support in 24/7 care coverage including Nights/Weekends/Holidays
• Perform patient assessments and maintain documentation
• Participate in the interdisciplinary plan of care and quality improvement activities
• Maintain professional standards and patient confidentiality
