Sentara Hospice Services – South Boston, VA is seeking a Licensed Practical Nurse to provide direct, one-on-one home health care. Hires must have an LPN license for VA, a trade school nursing credential, and BLS within 90 days. Prior hospice experience is preferred.

Sentara Health is seeking to hire a qualified individual to join our team as a Licensed Practical Nurse.

Overview:

The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one‑on‑one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high‑quality, compassionate care that improves patient well‑being.

Anticipated Status/Shift:

Part‑Time (24 hours/week)

Night Shift (12AM-8AM)

Position Location:

Sentara Hospice Services – South Boston, VA (travel can include up to a 50-mile radius for position)

Hourly Range: $22.39-$37/hour

Position Requirements:

Education

• Trade School Graduate for Nursing (Required)

Certification/Licensure

• LPN License in state of practice (VA) (Required)
• BLS (Required within 90 days)

Experience

• 2 years' experience (Required)
• Prior hospice experience (Strongly preferred)
