Job summary

Sentara Hospice in Chesapeake, VA is seeking a Licensed Practical Nurse (LPN) to join our home health care team. You will provide direct, one-on-one patient care, monitor health, administer treatments and medications, and collaborate with RNs and physicians to deliver high-quality, compassionate care.

The role is full-time on the night shift (12 AM–8:30 AM). Candidates must hold a VA LPN license and complete BLS within 90 days. A strong preference for hospice experience is noted.

Qualifications

• LPN license required in VA state.
• BLS certification required within 90 days.
• Two years hospice experience preferred.

Responsibilities

• Provide direct, one-on-one in-home patient care.
• Monitor patient health and administer treatments and medications.
