Job summary

Baylor Scott & White Health in Texas seeks a Hospice Registered Nurse to coordinate patient care with a patient-centered approach, develop care plans, and track patient progress.

In this role you will evaluate nursing practices, support quality improvement, educate patients and families, and collaborate with a multidisciplinary team to deliver compassionate end‑of‑life care. You will maintain privacy and respect autonomy while guiding ethical decisions.

Qualifications

• Associate's degree required; RN license preferred.
• One year of relevant nursing experience required.
• BLS certification within 30 days of hire.
• RN license in good standing and active licensure.

Responsibilities

• Develop, implement, and revise care plans for patient outcomes.
• Evaluate nursing practice quality, safety, and effectiveness.
• Create a safe, helpful environment for patients, families, and team members.
• Respect and incorporate patient/family values into individualized care plans.
• Maintain patient privacy, autonomy, dignity, and rights in care delivery.
• Educate patients, families, and team members about care options and treatments.
• Collaborate with the care team to plan and coordinate care.
