Job summary

Baylor Scott & White Health is seeking a Hospice Registered Nurse to coordinate patient care with a patient-centered approach and to evaluate health responses. You will create care plans, track patient progress, and ensure compassionate, ethical nursing practice across care teams.

Your responsibilities include developing and revising care plans, evaluating care quality, and educating patients, families, and team members to support informed healthcare decisions while maintaining patient privacy

Qualifications

• Associate's degree.
• One year of relevant work experience is required.
• Must be a registered nurse (RN).
• BLS certification within 30 days of hire.

Responsibilities

• Develop, implement, and revise patient care plans to achieve outcomes.
• Evaluate nursing practice quality and drive evidence-based improvements.
• Create a safe environment and build therapeutic relationships with patients and families.
