Job summary

Baylor Scott & White Health in Texas is seeking a Hospice Registered Nurse to coordinate patient-centered care, develop and revise care plans, and track patient progress to ensure quality outcomes.

The ideal candidate holds an RN license, an Associate's degree, one year of relevant experience, and BLS certification within 30 days of hire; excellent communication, ethics, and teamwork are essential.

Qualifications

• Associate's degree.
• One year (1) relevant work experience needed.
• Must be a registered nurse (RN).
• (BLS) Basic Life Support certification within 30 days of hire.

Responsibilities

• Develop, implement, and revise care plan for patient outcomes.
• Evaluate the quality and effectiveness of nursing practice, including delicate indicators.
• Create a safe, helpful environment for patients, families, and team members. Build therapeutic relationships, manage pain, and set boundaries to succeed.
• Respect and incorporate a patient's and family's unique differences into an individualized plan of care.
• Maintain patient and family privacy, autonomy, dignity, and rights. Create a plan reflecting their values and goals.
