Qualifications

• Graduate of an accredited school of nursing and currently licensed as an RN in Florida.
• Hospice or homecare experience preferred.
• Strong communication, documentation, and nursing assessment skills.

Responsibilities

• Respond to patient census and family inquiries after hours.
• Perform urgent clinical and psychosocial assessments.
• Provide pain and symptom management and maintain a safe environment.
• Document care thoroughly and timely using appropriate records.
• Collaborate with physicians and IDG for continuity of care.
• Coordinate after-hours admissions, referrals, and vendor/DMEs as needed.
