Job summary

Affinity Health Management seeks an On-Call Hospice RN to respond to patient census and family inquiries during nights, weekends and holidays. You will provide urgent clinical and psychosocial interventions, focusing on pain and symptom control with coordination across the hospice team.

The role requires independent assessment, documentation, and collaboration with the Medical Director and IDG to ensure quality patient care. A Florida RN license is required.

Qualifications

• Graduate of an accredited nursing program.
• Current unencumbered Florida RN license.
• Two years hospice/homecare/medical-surgical/oncology experience preferred.
• Valid driver’s license with insured vehicle.

Responsibilities

• Respond to Hospice patient census and family inquiries after hours.
• Conduct urgent physical and psychosocial assessments and determine interventions.
• Provide pain control and symptom management; ensure safe environment.
• Document care thoroughly and maintain timely records.
• Collaborate with physicians and IDG; coordinate with vendors as needed.
