Job summary

Our Hospice of South Central Indiana in Columbus, IN is seeking a Night Shift Registered Nurse (RN) to support an inpatient facility. You will work with a caseload of patients and collaborate with Nurse Practitioners, Physicians, Aides, Chaplains, and Social Workers to navigate each patient’s journey.

This 3x12 shift runs 7 pm to 7:30 am, with every other weekend. The role emphasizes comprehensive assessment, plan of care development with the IDG, discharge planning, education for patients and

Qualifications

• ASN or BSN degree required; enrolled in accredited program acceptable.
• Licensed Registered Nurse in Indiana.

Responsibilities

• Provide nursing care to hospice inpatients and assess patient/family needs.
• Serve as case manager; manage visits, supplies, and documentation.
• Teach and counsel patients and families on pain, symptom control and care techniques.
• Provide discharge planning for patients leaving the inpatient center.
• Develop and adjust the plan of care with the Interdisciplinary Group (IDG).
• Coordinate with IDG members and other staff to ensure quality care.
• Promote hospice benefits and educate referrals.
• Maintain records per organizational policies and procedures.
• Perform essential duties with reasonable accommodations if needed.
