Our Hospice is hiring a Night Shift Registered Nurse (RN) for our 14 bed Inpatient Facility in Columbus, IN. An RN works with a caseload of patients and the support of Nurse Practitioners, Physicians, Aides, Chaplains, and Social Workers.

Qualifications

  • ASN or BSN in nursing; or currently enrolled in accredited program.
  • 1–3 years health care experience preferred.
  • Licensure: Indiana RN; CPR certified.
  • Valid Indiana Driver’s License.
  • Hospice/palliative care knowledge and principles.
  • Strong communication and education skills.

Responsibilities

  • Provides nursing care to patients in the Hospice Inpatient Facility and conducts comprehensive assessments.
  • Acts as case manager with responsibility for assigned patients and resource management.
  • Educates and counsels patients and families on care techniques, pain control, and symptom management.
  • Conducts discharge planning for patients returning to their residence.
  • Develops and updates individualized plans of care in collaboration with the IDG and coordinates with the team.

Our Hospice is hiring a Night Shift Registered Nurse (RN) for our 14 bed Inpatient Facility in Columbus, IN. An RN works with a caseload of patients and the help of Nurse Practitioners Physicians, Aides, Chaplains, and Social Workers. Our patients and families are in the most important time of their life and need a skilled nurse to help navigate their pathway. In this position, every day is a little different.

Shift Available:

Nights: 3x12s and every other weekend; 7 pm - 7:30 am

An RN at Our Hospice will:

  • Provides nursing care to patients/families in the Hospice Inpatient Facility. Completes assessment of the patient/family needs including physical emotional, and bereavement status.
  • Performs as case manager maintaining responsibility for assigned patients in a cost-effective and quality manner. Manages time, reimbursement of visits, pharmacy and medical supplies, and durable medical equipment.
  • Teaches, supervises and counsels’ patient/family care techniques including pain and symptom control, dietary instruction, use of equipment and observation techniques.
  • Provides Discharge Planning for patients leaving the Inpatient Center and returning to the place of residence.
  • Develops and implements the plan of care for each patient in collaboration with the patient, family and Interdisciplinary Group (IDG). Evaluates each patient/family as directed and makes appropriate changes to the plan of care.
  • Communicates with and updates other IDG team members as needed to coordinate and ensure quality care (e.g. Social Services, Home Health Aides, Volunteers, and Medical Director).
  • Promotes the Hospice benefit and provides education to all potential referring customers including but not limited to patients and family members, case managers, nursing facilities administrators and staff, physicians and physician office staff, and community groups.
  • Maintains accurate records in accordance with the supervising organization’s policies and procedures in a timely manner (e.g. initial plan of care, comprehensive plan of care, updates to the plan of care, daily documentation requirement, discharge summaries, death summaries).

QUALIFICATIONS

  • Education: Associated Degree (ASN) required; Bachelor’s Degree in Nursing Science (BSN); or currently enrolled in an accredited program preferred.
  • Experience: One to three years’ experience in health care preferred.
  • Licensure: Valid Indiana Driver’s License.
  • Licensed Registered Nurse in the State of Indiana.
  • CPR Certified.
  • Skills: Strong clinical assessment, critical thinking, and decision-making skills with the ability to identify changing patient conditions and implement appropriate interventions.
  • Comprehensive knowledge of hospice and palliative care principles, including pain and symptom management, end-of-life care, and supportive services.
  • Excellent communication skills with the ability to effectively educate, counsel, and support patients, families, caregivers, and healthcare professionals.
  • Demonstrated ability to develop, implement, evaluate, and revise individualized plans of care in collaboration with the interdisciplinary team.
  • Strong case management and care coordination skills with the ability to prioritize, organize, and manage a complex patient caseload effectively.
  • Knowledge of Medicare Hospice Conditions of Participation (CoPs), state regulations, accreditation standards, and hospice compliance requirements.
  • Ability to establish and maintain positive working relationships with patients, families, physicians, facility staff, referral sources, and community partners.
  • Proficient in electronic medical record (EMR) systems, point-of-care documentation, and Microsoft Office applications. Ability to accurately document assessments, interventions, care plans, and clinical outcomes in a timely manner.
  • Strong time management, organizational, and self-direction skills while working independently in multiple care settings.
  • Ability to recognize psychosocial, emotional, cultural, and spiritual needs and incorporate those needs into patient and family-centered care.
  • Skilled in patient and caregiver education related to disease progression, medication management, symptom control, safety, and end-of-life care.
  • Ability to analyze clinical information, solve problems, and respond appropriately in urgent or crisis situations.
  • Demonstrated commitment to collaboration, professionalism, compassion, integrity, and excellence in hospice care.
