Job summary

Our Hospice of South-Central Indiana is seeking a Registered Nurse Case Manager (RNCM) for the Columbus area. You will visit patients in their homes or facilities, collaborate with an interdisciplinary team, and manage a patient caseload with on-call rotation.

Hours are 8 am - 4:30 pm, Monday through Friday, with some flexibility. Requirements include an active Indiana RN license and 1–3 years of healthcare experience; ASN/BSN preferred.

Qualifications

• Current Registered Nurse license in Indiana.
• One to three years' experience in health care preferred.
• Interest and ability in providing patient care in homes and facilities.
• Ability to travel via your own vehicle.
• Valid Indiana driver's license and car insurance.

Responsibilities

• Make routine Hospice nursing visits to patients in the home or facilities.
• Manage assigned patients' plan of care with IDG input and updates.
• Educate patients and families on care techniques and symptom control.
• Participate in on-call service to ensure 24/7 availability.
• Maintain accurate records per organization policies and procedures.
