Idaho Home Health and Hospice of Treasure Valley, part of the LHC Group within the Optum family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care after regular business hours. The role emphasizes triage by phone, afterhours crisis response, and timely home visits to assess needs and eligibility.

Requirements include an unrestricted RN license in the state, a valid driver's license with access to a vehicle, and CPR certification or the ability to obtain it within 90

Formación

• Current unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsabilidades

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to crisis calls from patients and families after hours with urgency.
• Perform afterhours nursing evaluations and admissions within 48 hours of referral when assigned.
• Conduct afterhours visits including admission, death, recertification, and routine visits.
• Assess and document patient/family needs and adjust care plans as needed.
