Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. You will cover nights, weekends, and on-call periods, ensuring continuity of care and strong clinical oversight for assigned patients.

Responsibilities include assessments, admissions, routine visits, death visits, triage calls, and updating the plan of care in collaboration with physicians and the IDG.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Support IDG processes through chart reviews and plan of care updates.
• Educate patients and families on disease progression and symptom management.
• Maintain accurate clinical documentation and care plans.
• Coordinate scheduling and triage for visiting clinicians.
