Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care during nights, weekends, and on-call hours in Texas. The role includes direct patient care, assessments, and coordination with the IDG to ensure seamless care delivery.

Responsibilities feature plan-of-care development with physicians, accurate documentation, and leadership for field staff while maintaining compliance and HIPAA standards.

Qualifications

• Knowledge of hospice principles, Medicare/Medicaid regulations, and best practices.
• Ability to conduct thorough patient assessments and develop individualized care plans.
• Strong communication with patients, families, physicians, and IDG members.

Responsibilities

• Provide direct in-person care during on-call, weekend, and after-hours shifts.
• Coordinate plan of care updates with physicians and IDG members.
• Educate patients and families on disease progression and symptom management.
