Gentiva is seeking an Afterhours RN Care Manager to ensure high-quality hospice care outside standard hours. You will provide direct patient contact during nights and weekends, coordinate with the IDG, and manage care plans to support patients and families.

Qualifications

• RN license in state of employment
• Graduate of an accredited nursing program
• Minimum 1 year RN experience in acute care, oncology, long-term care, hospice, or home health
• Must meet state-specific requirements

Responsibilities

• Provide direct in-person care during after-hours, weekends, and on-call shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Coordinate care with physicians and the IDG; update plans of care.
• Maintain accurate clinical documentation and care plans.
• Support scheduling, triage, audits, and compliance activities.
• Lead field staff and participate in required meetings/trainings.
