Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care during nights, weekends, and on-call hours. The role includes assessments, admissions, routine visits, and collaboration with physicians and the IDG to maintain the plan of care.

You will manage documentation, coordinate scheduling, and participate in regulatory compliance while upholding compassionate care standards in a fast-paced environment.

Qualifikationen

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, hospice, or home health.
• Must meet any state-specific requirements.

Aufgaben

• Provide direct after-hours hospice care during nights, weekends, and on-call hours.
• Assess patients and families and coordinate plans of care.
• Collaborate with the IDG to ensure continuity of care.
• Maintain accurate clinical documentation and plan of care updates.
• Educate patients and families on disease progression and symptom management.
• Lead and supervise field staff during visits and on-call shifts.
